1. Complete your survey w/ at least 40 queries |

2. Complete the informed consent form using either the
College's form or the form found on Blackboard for the course |

3. Add a short (100 words MAX) written introductory statement
that goes on the top of your survey |

participating & the survey is anonymous |

b. A brief statement
describing the topic |

| |
c. Cautions,
if needed (This survey may cause you anxiety... etc. If it is too
much, STOP) |

d. You were
asked to sign an informed consent form |

e. Diligence
& time |

4. Oral statement: Follows the same form
as above, but may be longer, more explanatory |
